Treatment: to provide, coordinate, and manage your care. Payment: to bill and obtain payment for services. Health care operations: for quality, training, and administration. We may also use PHI to remind you of appointments and to share information as otherwise permitted or required by law.
Your rights
You have the right to inspect and copy your records; to request amendments; to an accounting of certain disclosures; to request restrictions and confidential communications; to a paper copy of this notice; and to be notified of a breach of unsecured PHI.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
