What BMI does not tell you
BMI is a screening tool, not a diagnosis. It uses only height and weight, so it can be misleading:
- Muscle counts as weight. Very muscular people can register as “overweight” while carrying little body fat.
- It ignores where fat sits. Fat around the abdomen carries more metabolic risk than fat on the hips or thighs — that’s why we also calculate your waist-to-height ratio. Keeping your waist under half your height is a widely used guide.
- Thresholds differ by ancestry. The World Health Organization notes that people of South and East Asian descent may face increased risk at a lower BMI (around 23) than the standard 25 cut-off.
- Age and sex shift body composition. Muscle mass typically declines with age, so an identical BMI can mean different things at 25 and at 65.
- It says nothing about your labs. Blood pressure, blood sugar, cholesterol and family history all matter — and none of them appear in a BMI.
