Do I Qualify for Weight-Loss Medication?

Medically reviewed by Dr. Ehtesham Ghani, Internal Medicine & Bariatric Medicine (ASBP). Last reviewed June 2026.

Assessing eligibility for weight-loss medication

One of the most common questions we hear at ThinFast MD is simple: “Do I qualify for weight-loss medication?” The honest answer is that qualification is a medical decision, not a checkbox you complete online. It depends on your weight, your health history, your lab work, and which medication is being considered. Below, our medical team walks through the general criteria used in clinical practice and explains how qualification actually works at our four Illinois locations in Hinsdale, Arlington Heights, Brookfield, and Rockford.

What are the general criteria to qualify for weight-loss medication?

For FDA-approved anti-obesity medications, prescribing guidelines generally use body mass index (BMI) as a starting point. According to FDA labeling for medications approved for chronic weight management, such as Wegovy (semaglutide) and Zepbound (tirzepatide), they are indicated as an adjunct to a reduced-calorie diet and increased physical activity for adults with a BMI of 30 or greater (obesity), or a BMI of 27 or greater (overweight) when at least one weight-related condition is also present. Common weight-related conditions named in the prescribing information include high blood pressure, type 2 diabetes, and obstructive sleep apnea.

BMI is only the entry point. Qualification also requires a medical evaluation to confirm that a medication is appropriate and safe for you specifically. These medications are intended to be used with diet, exercise, and ongoing medical supervision, not as a standalone fix. Individual results vary.

Why does qualifying require an in-person medical evaluation?

BMI alone cannot tell a physician whether a given medication is safe for you. That is why ThinFast MD relies on an in-person physician exam rather than a quick telehealth-only questionnaire. At your consultation, Dr. Ghani and our team review your medical history, perform a physical exam, and order appropriate labs. Depending on your situation, that may include reviewing your metabolism, screening for thyroid conditions, and obtaining an EKG before considering certain medications.

This is a meaningful difference from mail-order and telehealth-only competitors that ship medication after a brief online form. An in-person history and physical, labs, and EKG when indicated allow your physician to match the right tool to your body and to catch the contraindications described below before a prescription is ever written.

What conditions might make someone qualify even with a lower BMI?

Many patients qualify at a BMI of 27 or higher because they also live with a weight-related health condition. ThinFast MD supports patients managing PCOS, type 2 diabetes and prediabetes, thyroid conditions including Hashimoto’s, and the metabolic changes of menopause and being over 50. We also offer dedicated programs for men’s weight loss, an adolescent program, and pre- and post-bariatric support. If a weight-related condition is part of your picture, it may broaden the medication options your physician can consider, which is one reason a full evaluation matters.

What can disqualify someone from weight-loss medication?

Contraindications exist, and they are taken seriously. For GLP-1 and GIP/GLP-1 medications such as semaglutide and tirzepatide, the FDA prescribing information carries a boxed warning, and these medications are contraindicated in people with a personal or family history of medullary thyroid carcinoma (MTC) or with Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). They are also not recommended during pregnancy. For appetite suppressants such as phentermine, a short-term sympathomimetic, the prescribing information cautions against use in patients with significant cardiovascular disease, uncontrolled high blood pressure, hyperthyroidism, a history of drug abuse, and certain other conditions.

This is exactly why qualification is individualized. Pregnancy, certain medication interactions, and other health factors can also affect eligibility. Your physician determines which option, if any, is clinically appropriate for you. We do not provide individualized medical advice online, but at your visit we review your full history before recommending a plan.

Which medications might I qualify for at ThinFast MD?

Once you qualify, your physician selects from a range of options based on your health profile. Among the semaglutide and tirzepatide options, Wegovy and Zepbound are FDA-approved specifically for chronic weight management, while Ozempic and Mounjaro are FDA-approved for type 2 diabetes; your physician will determine, within clinical judgment, what is appropriate for your situation. Options may also include compounded semaglutide and appetite suppressants such as phentermine, phendimetrazine, diethylpropion, and metformin. We also offer OPTIFAST and OptitrimMD meal replacement, lipotropic and B12 injections, and nutrition counseling to support your results. If you want to understand specific options, our pages on tirzepatide weight loss and GLP-1 semaglutide explain how each works. Every medication is used with diet, exercise, and medical supervision, and individual results vary.

How much does weight-loss medication cost, and is it covered?

Cost and insurance coverage vary widely by medication, by plan, and by your individual situation. Rather than quote a number that may not apply to you, ThinFast MD reviews pricing and any insurance considerations directly at your consultation, so you get accurate information for your specific plan and treatment path. We serve patients across Illinois from our Hinsdale, Arlington Heights, Brookfield, and Rockford clinics, and we also offer telehealth and online consults for follow-up care.

Frequently asked questions

What BMI do I need to qualify for weight-loss medication?

FDA labeling for weight-management medications like Wegovy and Zepbound indicates a BMI of 30 or greater, or 27 or greater with a weight-related condition such as high blood pressure, type 2 diabetes, or sleep apnea. A medical evaluation confirms whether you qualify.

Can I qualify if my BMI is under 27?

Standard FDA prescribing criteria for anti-obesity medication generally begin at a BMI of 27 with a weight-related condition. Your physician evaluates your full health profile to determine appropriate options for your situation.

What would prevent me from qualifying?

Contraindications exist. GLP-1 and GIP/GLP-1 medications carry an FDA boxed warning and are contraindicated in people with a personal or family history of medullary thyroid carcinoma or MEN 2, and are not recommended in pregnancy; phentermine is not advised for people with significant cardiovascular disease or uncontrolled high blood pressure. Your physician reviews these at your visit.

Do I need an in-person visit to qualify?

At ThinFast MD, qualification includes an in-person physician exam with history, physical, labs, and an EKG when indicated, rather than an online-only form. This helps ensure any medication prescribed is safe and appropriate for you.
