How is ThinFast MD different from a TRT or men’s hormone clinic?
Many men’s clinics lead with testosterone replacement therapy and treat weight as an afterthought. We do the opposite. ThinFast MD is a true medical weight-loss practice, and we do not offer or position testosterone therapy as a weight-loss treatment. Our medical director, Dr. Ehtesham Ghani, is board-certified in Internal Medicine and a member of the American Society of Bariatric Physicians, and the focus stays on the science of fat loss and metabolic health. What does a men’s weight loss evaluation include?
Unlike mail-order and telehealth-only services that ship medication after a brief online questionnaire, ThinFast MD begins with an in-person physician exam. That typically includes a history and physical, lab work, an EKG when appropriate, and a review of your metabolism and any conditions that affect weight, such as type 2 diabetes, prediabetes, thyroid issues, or low energy patterns. This is also how we screen for the medical conditions that make certain medications unsafe for some people before anything is prescribed. It is how we catch what a questionnaire misses and tailor the program rather than handing out the same protocol to everyone.
What weight loss medications might be part of a men’s plan?
When clinically appropriate, your physician may consider GLP-1 medications such as semaglutide (brand Wegovy or Ozempic) or the GLP-1/GIP medication tirzepatide (Zepbound or Mounjaro), each used with diet, exercise, and medical supervision. In the SURMOUNT-1 trial published in the New England Journal of Medicine (2022), adults without diabetes taking tirzepatide lost an average of about 16 to 22.5 percent of body weight depending on dose over 72 weeks, alongside lifestyle changes. In the STEP 1 trial (NEJM, 2021), semaglutide produced a mean body-weight reduction of about 15 percent (-14.9%) over 68 weeks. These figures come from controlled clinical trials, and individual results vary. Are these weight-loss medications safe, and who should not take them?
These medications are effective tools, but they are not right for everyone, which is exactly why we start with an exam. According to FDA prescribing information, GLP-1 and GLP-1/GIP medications such as semaglutide and tirzepatide carry a boxed warning and are contraindicated for people with a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Appetite suppressants like phentermine are generally not appropriate for people with significant or uncontrolled cardiovascular disease, moderate-to-severe or uncontrolled high blood pressure, or hyperthyroidism. Your physician reviews your history, labs, and EKG to decide whether any medication is safe for you specifically, and medication is prescribed only when it is clinically appropriate.
