Which one leads to more weight loss?

In SURMOUNT-5, a head-to-head clinical trial published in the New England Journal of Medicine in 2025, tirzepatide produced a greater average weight reduction than semaglutide over the 72-week study period. That is meaningful data, but averages are not promises. Clinical trials measure groups of people under controlled conditions; your individual response depends on your starting weight, metabolism, other health conditions, how consistently you take the medication, and the lifestyle changes you make alongside it. Some patients do very well on semaglutide and have no reason to switch. The point of comparison data is to inform a conversation with your physician, not to declare a single winner for everyone. Do Wegovy and Zepbound have the same side effects?
The side-effect profiles are broadly similar because both are incretin-based medications. The most common reactions reported in the FDA prescribing information for each drug are gastrointestinal: nausea, diarrhea, constipation, vomiting, and abdominal discomfort, especially when first starting or increasing the dose. These effects are often more manageable when the dose is titrated slowly, a key reason medical supervision matters. Both medications carry a boxed warning regarding the risk of thyroid C-cell tumors based on rodent studies, and neither is appropriate for people with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia syndrome type 2 (MEN 2).
